Did you clear the code after cleaning the EGR? Did you replace the valve with a new one or used? Did you test it with a vacuum gauge?

You still need to reset the code to clear it out of the ECU, and double check your vacuum lines not sure if I saw where you said you did this, you may also wanna check hoses 3, 5, and 21,sometimes those are easy to mix up with the EGR hose at the MAP box.

Turning off the car doesn't clear the ECU memory. Remove the ACG fuse from the fuse box under the dash for 15-20 seconds. If the code comes back, you still have a problem.

Yeah, but it's still easier than crawling under the dash, at least, for my old ass it is!

My NOx wasn't quite over the limit here in AZ (my CO was over though) but here are my numbers and how they changed to after each step:

AZ allowed 2.50

1st test 2.30 --> cleaned EGR tube and valve, changed to new plugs
2nd test 1.42 --> changed O2 sensors
3rd test 0.64

Code 12 is the EGR lift sensor. Could simply be a bad sensor.

The lift sensor is connected to the valve, which is what you already replaced. Correct? You need to get a vacuum gauge and a vacuum pump to properly test the system.

On Honda (UK) you may download the workshop manuals for free:
cars->Honda owner->Parts and servicing->workshop manual

The sensor for the AGR valve lift is sitting on top of the AGR valve. It is a small box made of thin, silver sheet metal. Possible reasons for wrong sensor signal:
-vacuum does not built up on the AGR. If the AGR valve is supplied with vacuum at mid-load, check the diaphragm in the AGR valve for leaks. If no vacuum is present, check the hoses and the modulation valve in the control box
-AGR valve is stuck, clean with oil and move it manually
-defective lift sensor or missing supply voltage.
